WAIVER AND RELEASE OF LIABILITY

In consideration of being permitted to participate in the Walk to Bust Cancer (the "Event"), organized by National Breast Center Foundation d/b/a Breast Cancer Collective ("Breast Cancer Collective"), I acknowledge and agree as follows:

1. Assumption of Risk and Release

I understand that participation in the Event involves inherent risks, including but not limited to falls, contact with other participants, traffic, weather conditions, road or trail conditions, and other hazards that may result in serious injury, illness, disability, or death.

I voluntarily assume all risks associated with my participation in the Event.

To the fullest extent permitted by law, I hereby release, waive, discharge, and covenant not to sue:

• National Breast Center Foundation d/b/a Breast Cancer Collective;

• the Walk to Bust Cancer;

• the Event venue and property owners;

• Event sponsors and partners; and

• each of their respective directors, officers, trustees, employees, volunteers, contractors, agents, successors, assigns, and affiliates

(collectively, the "Released Parties")

from any and all claims, liabilities, demands, damages, losses, or causes of action arising out of or relating to my participation in the Event, including claims arising from the ordinary negligence of any Released Party, except where prohibited by applicable law.

2. Indemnification

I agree to indemnify, defend, and hold harmless the Released Parties from and against any claims, demands, actions, damages, liabilities, costs, or expenses (including reasonable attorneys' fees) arising out of my participation in the Event or any claims brought by or on my behalf.

3. Physical Condition

I certify that I am physically able to participate in the Event and have not been advised otherwise by a qualified medical professional. I agree to participate within my own physical limitations and to discontinue participation if I believe continuing would present a risk to my health or safety.

4. Medical Treatment

I authorize Event personnel to obtain emergency medical treatment for me if deemed necessary. I understand that I am solely responsible for any medical expenses incurred as a result of my participation in the Event.

5. Photography and Media Release

I understand that photographs, video recordings, audio recordings, livestreams, and other media may be captured during the Event.

I grant National Breast Center Foundation d/b/a Breast Cancer Collective and its designees the unrestricted, perpetual, worldwide, royalty-free right to use my name, image, likeness, voice, and appearance in any media, now known or later developed, for promotional, educational, fundraising, or other lawful organizational purposes without compensation or further approval.

6. Personal Safety

For my own safety and the safety of others, I agree to exercise reasonable caution while participating in the Event, including limiting distractions caused by headphones, mobile phones, or other electronic devices.

7. Entire Agreement

This Waiver and Release constitutes the entire agreement between me and the Released Parties concerning my participation in the Event and supersedes any prior oral or written representations. No modification of this Agreement shall be effective unless made in writing by National Breast Center Foundation d/b/a Breast Cancer Collective.

8. Severability

If any provision of this Waiver and Release is determined to be invalid or unenforceable, the remaining provisions shall remain in full force and effect.

9. Acknowledgment

I acknowledge that I have carefully read this Waiver and Release of Liability, fully understand its contents, understand that I am giving up substantial legal rights, and voluntarily agree to its terms. I understand that my electronic acceptance or signature has the same legal effect as my handwritten signature.
